Those patients with multiple or complex medications will benefit.

Those patients needing medications packaged daily, weekly or monthly.

Patients with newly diagnosed heart disease, diabetes or any other health disorder.

Patients newly discharged from the hospital or rehabilitation facility.

Prior to Initial Visit

Receive complete list of medications from physicians and caregivers.

At Initial Pharmacist’s Visit

Examine pill bottles for compliance.

Take first patient assessment to identify areas of possible intervention to improve medication adherence and quality of life.

Set up first medication tray after medications are completely reviewed.

Subsequent Visits and Management Services

Weekly delivery of medication trays.

Documentation of medication changes and new orders.

Monitor effectiveness of program by measuring therapeutic outcomes to identify new areas of need.

Periodically reassessing.

Assessments sent to family and physicians as requested for review.
