West End Pharmacy’s Medication Management Program provides an opportunity for our elderly and more challenged patients to be protected from medication related problems. Our goal is to improve quality of life, relieve stress caused by multiple medication therapies, prolong independent living and ultimately reduce healthcare costs to both the family and the healthcare industry.

An initial assessment is the first step in our caring program. The interview will review all medication-related needs with the patient: understanding, compliance, interactions and effectiveness. The visit will focus on the patient’s disease state and self monitoring practices. Issues in the home are also taken into consideration as they relate to drug therapy. Finally, the physician is consulted and medication therapy is reviewed.

After the initial assessment, the pharmacy will fill and deliver, on a weekly basis, a medication tray to the patient’s home! All medication refills and changes are handled by our well trained staff of pharmacists and certified pharmacy technicians. Medication therapy is documented and can always be sent to family and physicians as requested for review.
